Incident Response Data Collection

As a client of Gallop Technology Group, we must be aware of the insurance policy coverage and compliance/ regulatory requirements of your business.
In case of a cyber incident, we must know who your insurance company is expecting you to contact, and what needs to be done to minimize legal damages and further losses.

This information MUST be provided to us if you ever expect us to assist you in case of a cyber incident, without losing very valuable time during those stressed moments.

Please reach out to us if you have any questions about this form, and please make sure you contact your insurance provider to VERIFY the contact information and type of coverage you have with them, to confirm the CYBER policy is indeed in plan, and covers your company’s current needs.
